Youth Info and Consent

 
 
Does your child have any communication needs e.g. non-English speaker/hearing impairment/sign language/dyslexia. If yes please tell us what we need to do to enable him/her to communicate with us?
 
Does your child participate in religion or spiritual practice? Please tell us what it is. What do we need to know to ensure your child’s preferences/needs are met e.g. are there any dietary guideline/dress codes we need to follow or support?
 
At times the Club may wish to take photos or videos of the team or individuals in it. We adhere to The FA Guidelines to ensure these are safe and respectful and used solely for the purposes for which they are intended, which is promotion and celebration of the activities of the Club and for training purposes.

If your child has a serious long-term medical issue, including but not limited to: cardiovascular problems, neurological episodes, respiratory issues; we must receive a medical note from your child’s GP declaring them fit to participate in the activities which fall under the care of Harrogate Town AFC.
Participants who require essential medical aids, including but not limited to: inhalers, EpiPen’s, hearing loop; aids must be brought to all activities which fall under the care of Harrogate Town AFC. Failure to do so will result in them being unable to participate or be under our care
I give consent for my son/daughter to participate in Harrogate Town Association Football Club’s events and agree to the conditions outlined above. I accept that it is my responsibility to inform the Club directly of any changes to the details recorded on this form.
I have read the above information and have stated all my child’s previous medical conditions. I take it upon myself to update Harrogate Town Association Football Club regarding any changes to my son or daughter’s health. In the event that my son/daughter is injured whilst playing football whilst representing Harrogate Town and I cannot be contacted on the details I have provided, I hereby give my consent for my child to receive the relevant medical attention and transferred to hospital if required.
